Description of the
service delivery process |
The
duplicate licences are issued in accordance with the Republic of Lithuania
Law on Alcohol Control, 1995, no 44-1073, and the Government of the Republic
of Lithuania on 20 May 2004. Resolution No. 618 on the rules for the licensing
of wholesale and retail trade in alcohol products and on the approval of the
rules for retail sale of alcoholic beverages in trade and catering
enterprises ('), 2004, no 84-3050 '. |

Information and
documents to be provided by the person |
A company
which has lost or damaged a licence and who wishes to obtain a duplicate
licence must submit an application stating the following details: Company
details, telephone number, e-mail address (if the company has one). If a
duplicate licence is issued due to a damaged original licence, the
undertaking must submit the original of the damaged licence together. As well
as placing a payment order with bank tags or a voucher that the state levy
has been paid for. |

Appeal procedure for
the service provider's action/omission |
Action
(omission) can be appealed in accordance with the procedure laid down by the
Law on the legal Proceedings of the Republic of Lithuania for the Regional
District Administrative Court of Klaipeda Chamber of (Galinio Pylimo g. 9,
Klaipėda). |
